- 1. Submit a requisition form for a certified copy of birth, marriage, or death certificates for apostilles and authentication. Previously certified copies of vital records aren't accepted. 2. Fill out the "Lt. Governor’s Apostille or Document Certification Form." 3. A separate money order or cashier's check of $1, payable to the "Lt. Governor's Office." 4. An additional money order or cashier’s check for fees relevant to birth, marriage, or death record requests, payable to the "Hawaii Department of Health."
