- Original, notarized, or certified record. A properly notarized document will have four elements in the notarial certificate: The Venue – the state and county where the notarization was performed; the Statement of Particulars – a sentence that contains the date of notarization, the type of notarial act, and a reference to the signer or the type of document (depending on the type of notarization); the notary’s official signature; and the notary’s official stamp. Copies of official records must be issued within the last five years. If you have a birth, death, marriage, or court record that was issued more than five years ago, you must obtain a new certified copy from the appropriate state or county office
