    I recently submitted a request online to have my birth certificate mailed to me after my mother was unable to locate it. Due to my work schedule conflicting with the department’s hours, I thought this would be the most convenient option. Unfortunately, with the holiday delays, I didn’t receive the certificate within the 2–3 business days I had hoped. On July 9, I made a last-minute attempt to visit the department in person, as I’m preparing for an upcoming cruise and needed the birth certificate urgently. This was the only day I could possibly make it—but I arrived just after closing. Thankfully, Anita Phillips was still there, and she went above and beyond to help me. She kindly took the time to issue the birth certificate despite the office being closed. Her understanding, patience, and exceptional customer service meant the world to me during a stressful moment. I truly hope you recognize Anita for her dedication and kindness. Employees like her make a huge difference, and I sincerely believe she deserves a raise or special recognition for her outstanding service.

    The scheduling appointments for vaccinations is the worst I've seen for a business. It's always a true struggle. I have not tried the other areas of the department. The Building is relatively clean. Staff is friendly in person. Sometimes they run out of certain types of travel vaccine so be sure to ask before you show up for an appointment.

    I haven’t made it to the location yet, but I have spoken with them on the telephone a couple times to find out exactly what was needed and every time I spoke to them, they were kind, helpful and very caring. I would’ve given them a five star, but like I said, I have not fulfilled my mission yet to get a copy of my son’s birth certificate, but if it is as easy going as it has been on the phone, I believe that they will get and deserve five stars and beyond.

    They are really nice until you say you are confused and have a question, doesn't matter if you're calm and polite the whole time. They will get defensive and tell you their supervisor isn't here (I didn't ask for one just asked if she could explain what was happening) then give you a number in Jacksonville to call and shut the door in your face.
