With the health code, you always have to be aware that taking into account safety is the number one priority here. These folks are responsible for investigating problems like stores' freezers not working properly putting the customer at risk if the food starts to spoil. A person answering the phone and taking information had a very off-hand attitude. One has to ask their name; it is not volunteered. They may not tell you that your name and number will be included in their report. They will ask you for your name and phone number. You can tell them that you do not wish your name and number in the report. Then one has to rely on their integrity to follow your instructions. They do say that they would like it in case they need more information.
