    Bad customer service at reception. If you plan to go here make sure you know excatly what to do before you get here, so the employees don't actually have to work, greet you, or help you make an apppointment. Washington County's finest what else is there to say.

    I have went to the Florida Department of Health in Washington County, Chipley, Florida for DOT Medical Certification for the past 5 years. I DO have another primary care physician as well as specialists I follow up with yearly as needed but they do not handle the DOT Medical Certificates. The past 4 years Julia Tice has been the medical professional that issued the DOT medical certification. Last year and this year for some reason she has become very confrontational, abrupt, overall less professional as well as annoyed that I only come in once a year. She was also in disagreement with the specialists I see regarding my medications and care. (I followed up with specialist and her diagnosis was wrong!) I will not be returning here for DOT physicals in the future. I did voice my specific issues to the receptionist and submitted a survey as she suggested. The receptionist handled my complaint with empathy and grace which was very appreciated at a time when I was very angry and frustrated. I received a call from the office manager shortly after returning home and she was also very professional and empathic regarding my complaints and indicated she would follow up with the appropriate people. If I could I would give the office staff and nurse a 5 star rating but Julia Tice would only receive 1 star rating.

    Refused to see my child for a physical without birth certificate. They've been given a copy twice AND transferred shot records from Alabama twice. Said they couldn't prove I was her mother and canceled my appointment. Go anywhere else but here!
