    I went today to get my birth certificate again since I didn’t have an actual one. I had called beforehand just to check on what was needed for me to bring and I spoke with Mandy I believe. She was very helpful and pleasant and friendly. In person there were two ladies and I’m sure one of them was Mandy but they were both great during the whole process. Thank you again for being kind and understanding.

    It's not too bad, I went in at a busy time to get shot records. And they wanted me to pay 5$ for them to print out a piece of paper, greatful they did it and understand they have to make money some how. The wait was bad when I went in, but that's because immunizations is always pretty busy. Still great service though. If you need shot records, don't call you should walk in. Calling is a nightmare.

    I am going to Taiwan & Thailand soon & needed to get the accelerated Hep A/B vaccination schedule (series of 3 shots within 21 days). Everything was easy to schedule, my appointments were always on time and the injection sites were never sore. Everybody was professional and super friendly - especially Sayonara who gave me 2 of the 3 shots. If you need any vaccinations, I'd highly recommend going to Davis County Health Department

    Impossible to get help! I am submitting this complaint due to ongoing and worsening mold issues in my apartment located in Layton, Utah. Despite notifying the owner the response was inadequate—someone was sent to wipe down the mold with water and paint over it without addressing the underlying cause. Since then, I have discovered multiple new areas of visible mold throughout the unit. I conducted mold tests using petri dishes, which revealed heavy mold growth in several locations, indicating dangerously high levels of airborne spores. I am also experiencing related health symptoms, including nasal congestion, sore throat, and eye irritation. I am concerned for my safety and well-being and request that the Davis County Health Department investigate the property and assess whether the living conditions meet health standards.
