The woman who recently issued a death certificate I needed was a pathetic individual. Old blonde woman with fake eyelashes. Would give zero stars if I could

I am not very impressed with the health department. My daughter is needing to get a learners permit and when I was issued her birth certificate her name was spelled wrong on it. When I went today to get a corrected one I was charged $15 even though they made a mistake and spelled her name wrong. I don't believe I should have been penalized for their mistake as I would not have needed another copy of the certificate if her name had been spelled correctly on the original one. On top of that when I was complaining about having to pay for their mistake the employee stated "You know I don't have to give this to you." Not very professional.

Just had to run in and pick up some paperwork. The employee met me and was very nice.

I ordered a replacement birth certificate and when I received it, it was not mine. They sent me someone else's which is a huge potential for identity theft. When I called them they refused to mail me mine until I sent the wrong one back (even though I already paid for it). Jammie Bane is very unprofessional and a liar. I was told once they received the wrong one that they would overnight mine. I overnight them the old wrong one, took a week to get mine.
